What Causes Sagging Skin

What Causes Sagging Skin

A softening jawline, or cheeks that sit lower than they used to, is one of the most common concerns clients raise. Skin sags when the structures that hold it up change faster than the skin can adapt. Collagen and elastin decline, the fat pads beneath the skin thin and shift downward, and the facial bones under them slowly recede, so the skin ends up draped over a smaller, lower framework than the one it originally fit.

Collagen explains part of this, and it is the part topical products can influence. It does not explain why sagging follows the particular pattern it does.

The Four Layers Behind Sagging Skin

Facial support comes from four layers, and each one changes on its own schedule.

Skin: Collagen is one of the primary structural proteins that helps support skin firmness, and elastin contributes to the skin's ability to stretch and return to its original shape. Production of both slows with age, and existing elastic fibers gradually fragment.

Fat: The face holds distinct fat compartments rather than one even layer, as Rohrich and Pessa first mapped in 2007. A 2021 study in Plastic and Reconstructive Surgery tracked the same patients’ CT scans more than a decade apart and found that deep midface fat shrank by about 18% and superficial fat by about 11%. That uneven loss can help explain why the midface looks hollow while the jawline looks heavier.

Muscle: Facial muscles lose tone over time, and the connective tissue anchoring them to bone becomes less taut.

Bone: A 2026 review in JPRAS Open, which pooled studies published from 2020 to 2023, found distinct patterns of bone resorption around the eye sockets, the nose, and the upper jaw. It reported that the eye socket enlarges by roughly 15 to 20% by the seventh decade and that the height of the maxilla, the bone supporting the midface, decreases by 8 to 15%. As bone recedes, the soft tissue above it is thought to have less to hold onto.

Topical products work on only one of those four layers, which is why they cannot address advanced sagging on their own.

Why Does Skin Sag With Age?

Aging drives the process on two tracks at once.

The first is intrinsic, meaning it happens regardless of how you live. Collagen production slows, elastin fibers fragment, and the fat and bone changes described above proceed quietly over decades. This track is largely set by genetics.

The second is extrinsic, driven by what the skin is exposed to, and it is the track your daily habits can influence. Ultraviolet exposure is the dominant extrinsic factor.

In a 2013 study of 298 Caucasian women aged 30 to 78, published in Clinical, Cosmetic and Investigational Dermatology, Flament and colleagues concluded that UV exposure appears responsible for about 80% of visible facial aging signs in the women studied, although sagging itself tracked more closely with age. UV radiation breaks down existing collagen and interferes with the production of new collagen.

What Makes Skin Sag Faster?

Several sagging skin causes can accelerate the process, and most of them are within your control.

  • Unprotected sun exposure

  • Smoking, which raises levels of enzymes that break down collagen and elastic fibers and impairs new collagen production

  • Significant or rapid weight loss, which can leave skin covering less volume than before

  • Repeated weight cycling, which may stretch and relax the skin in turn

Gravity is often blamed on its own. Its role is real but secondary, because it pulls on tissue whose anchoring has already weakened rather than causing that weakening.

Why Is My Skin Sagging Earlier Than Expected?

When sagging appears ahead of schedule, a few factors usually account for it.

What you notice

What may be contributing

Gradual change from your 30s onward

An intrinsic timeline that runs early, often with a genetic component

Looseness after losing a noticeable amount of weight

Reduced volume beneath skin that has not yet adapted

Sagging alongside uneven tone and visible sun damage

Cumulative ultraviolet exposure

Looseness that appeared over weeks rather than years

Something other than aging, worth discussing with a physician

Your esthetician can help narrow this down during a professional skin analysis.

Where Does Sagging Show First?

Softening usually appears along the lower face first. The prejowl area of the jaw is a common site of bone resorption, and it also sits where the jowl fat compartment tends to descend and collect. The Cheeks Luxury Skin Clinic guide to what jowls are and why they form covers that area in detail.

The neck and the area under the eyes also commonly show change, since both have thinner skin and less structural padding. On the body, the same loss of elasticity often shows on the upper arms and abdomen, particularly after weight change or pregnancy.

What Can and Cannot Be Changed?

Topical products work on the skin layer, where certain ingredients can support collagen production and improve how firm the skin looks. Professional treatments can reach the skin and muscle layers. Neither restores fat volume or changes bone, so for advanced laxity their effect is real but limited.

Daily sun protection is among the preventive steps with the strongest evidence. In a 2013 randomized trial in Annals of Internal Medicine, adults under 55 who used sunscreen daily were 24% less likely to show increased skin aging over about four and a half years. The Cheeks Luxury Skin Clinic guide to improving skin elasticity covers the supporting habits. Results vary based on the individual.

At Cheeks Luxury Skin Clinic, treatments that address the appearance of firmness include Exion Face, which uses radio frequency with targeted ultrasound and is designed to support collagen and elastin production; Emface, which pairs radio frequency with muscle stimulation and is designed to lift and tone; and the Non-Invasive Facelift, which uses microcurrent. Many clients notice gradual improvement over a treatment series, and results vary based on the individual.

Which of these suits you depends on whether your main concern is skin laxity, lost volume, or both. Your esthetician assesses this during a professional skin analysis using the Skin Instant© Lab, and no two treatment plans are exactly alike.

Frequently Asked Questions

Can sagging skin be reversed without surgery?

Nonsurgical treatment does not reverse the structural changes behind sagging, but it can improve how firm the skin looks. Mild to moderate laxity tends to respond best to nonsurgical treatment. Advanced sagging with substantial volume loss generally responds less, since nonsurgical options work on the skin and muscle layers without altering the bone beneath. A professional skin analysis shows which category you fall into, and results vary based on the individual.

Do facial exercises help with sagging skin?

The evidence remains limited. A 2025 systematic review in Aesthetic Surgery Journal Open Forum found some small studies reporting improvements in cheek skin and skin elasticity after 7 to 20 weeks of exercises, but concluded that the evidence is not yet strong enough to confirm they work. Facial exercises target muscle tone, which is only one of the four contributing layers, so at best they address part of the picture. They are unlikely to cause harm, but they do not replace daily sun protection.

At what age does skin start to sag?

Collagen production begins to slow in early adulthood, but visible looseness commonly appears from the late 30s or 40s. Timing varies widely with genetics and sun exposure history, and heavy sun exposure can bring it on earlier. A professional skin analysis can show where your skin stands now.

Does sagging skin from weight loss tighten on its own?

Skin may retract somewhat in the months after weight loss, particularly in younger skin and after gradual loss. How much depends on your age, how much weight you lost, how quickly you lost it, and how long the skin was stretched. Larger losses often leave some looseness, and results vary based on the individual.

Understanding Your Own Pattern of Sagging

Sagging skin reflects four layers changing together, with collagen as only one part of the story. Knowing which layer drives your particular pattern is what separates a treatment that helps from one that disappoints.

We can help you understand what is behind your skin's loss of firmness and which approach fits. Book a professional skin analysis with a licensed esthetician at Cheeks Luxury Skin Clinic in Williston, near Burlington, VT.
